Preferably using core comment module. Plus Inline Entity References (and possibly entity prepopulate) for changing the parent action when a comment is posted.

Futzed around for most of yesterday on this, trying to duplicate the way Drupal.org does it with project issues, via very dodgy workarounds. Then, well past sunset, I realised it actually wasn't worth the bother.

Marking this done. Would be nice to add a participant when a new person comment on an issue, but I'll leave that for later.
Likewise, changing status from the content edit form is a nice-to-have but can't neatly be done with the modules we have installed/enabled now. Must look into Workflow or Rules at some later date.
